Duplicated header in vcf output

After phasing, G-nomix spits out a phased vcf file.
Some of the header information comes once from the source (query) vcf and once from the G-nomix vcf writer:

This causes issues for downstream tasks.

Most of the input headers should probably be overwritten by the vcf writer.
