Team-formation issue strikes again: student accepts, but does not end up on team with inviter

rmalhot2 and dkannan tried to form a team for Program 1. rmalhot2 invited dkannan:

but dkannan cannot see the invitation:

The team listing in the instructor's view shows rmalhot on team Rubik (the intended team)

and dkannan on Team_3595:

This is the second time we've seen this situation today. In all of last semester, it never occurred, which suggests that it is the fault of a recent merge or bug fix.

Look at #2723 for a sql dump of the system before I fixed this by adding dkannan to the team.

This bug was evidently not triggered when creating teams for CSC 517 Program 2, for whatever that's worth.

While we didn't notice this during February, the only assignment there had topics, which means that teams were often formed before topics were selected. And it didn't occur in CSC/ECE 506 either, and they did have one non-topic assignment during February.
