Add HealthcareService sample data.

Assess and select the personas from Services Australia to be HealthcareServices using Organization & PractitionerRole
Assess initial gaps, add any additional HealthcareServices for 'minimum' set
Design and develop those HealthcareServices instances (assume csv file)
