Acquire of entity request permit not atomic with in-flight message registration

In ClientEntityManagerImpl, permit handling is in the general form:

acquire
register

vs

stop acquisitions
loop over registered

Registration of an in-flight message is not atomic with acquisition of the permit to register. This can lead to a client hang waiting for completion of a message that was never sent.
