Currently, the plugin only only enhances pages and posts. Update the plugin to enhance documents in all collections (including custom collections). Additionally, update the pre_render hook to match on pages and documents instead of pages and posts. The former combination matches pages and documents in all collections (including posts).

resolves asciidoctor#93 enhance documents in all collections
536e1e1 
- enhance documents in all collections
- add tests for custom document collection
- use default layout name that is based on name of collection
- rename collection to collection_name
