CHANGES:

Do not mutate the base encoder when using custom percent encoders.
This was a bug introduced in #147. (#156 @aantron)
Disable Travis CI tests and switch Win/Mac tests to GitHub Actions and
Linux ones to ocaml-ci (@avsm).
